How to be referred

All referrals must be made to the HAHAV Volunteer Co-ordinator.  Written and verbal referrals are accepted between 10:00 and 15:00, Mondays to Fridays.

Referrals are accepted from health and social care professionals.  In the case of a direct approach by the patient or carer, further information will be sought from the patient’s GP or district nurse.  

All new referrals will be contacted within one week.

An initial visit will be undertaken by a co-ordinator to assess the patient’s particular needs and those of their carers.  A HAHAV volunteer will be selected and then introduced, and during each visit the volunteer will provide the agreed service.
